This article describes the process from first proposals in the early 1990s to project completion many years later for seven large Swedish road and railway projects. The purpose is to find reasons for the massive cost overruns as well as explanations for why projects are brought to completion despite much higher costs than when the decision to build was made. Cost overruns are set in an institutional context to highlight the interplay among national, regional, and local policymakers. National investment programs are seen as promises by other parts of society, irrespective of whether project costs increase during the process toward procurement and implementation. Another aspect is that the infrastructure manager’s administrative framework currently makes it impossible to compare costs in contracts with final cost, meaning that there is no institutionalized learning process in place. Design preparations and the estimation of costs for new projects must therefore be done without an understanding of what has been working well in the implementation of previous projects. While Benefit-Cost Analysis (BCA) played no role in the planning of the seven projects, the article sends a stark warning that early cost estimates provide poor input for assessing project rate of return.
Birds of different feather flock together - genetic structure of Taiga Bean Goose in Central Scandinavia

During their flightless summer moult, Taiga Bean Geese Anser fabalis fabalis gather at communal moulting sites. Individuals from the Nord-Trøndelag breeding area in Norway have been observed to join with local individuals on moulting sites in Vilhelmina Municipality, Sweden. These two groups show distinct features in breeding habitat and migratory behaviour, but are they also genetically distinct? We used 12 microsatellite loci for genotyping 109 blood, feather and faecal samples from three sampling areas (Røyrvik in Norway and Stalon and Nästansjö in Sweden) to examine genetic diversity and structure. Clustering and Principal Coordinate analyses of all samples unveiled at least two distinct clusters, which were unevenly distributed over the sampling sites. Grouped by sampling sites, AMOVA and FST analyses showed that samples from the three sites differed genetically. These differences were larger between Røyrvik and Nästansjö than between Stalon and the other two. Relatedness was high among the Røyrvik samples. From our results we conclude that one of the clusters describes the Røyrvik breeding subpopulation, while the other(s) breed mainly in Sweden. Although these subpopulations simultaneously use the same moulting area in Vilhelmina, they appear to be ecologically, behaviourally and genetically distinct, in particular the Røyrvik sub-population. For goose conservation and management, we suggest that the Nord-Trøndelag (Røyrvik) subpopulation is considered a separate flyway management unit. Unravelling the Swedish sub-populations will need further study. For bird conservation is general, we suggest active genetic sampling for detailed population structure analyses and subsequent differentiated conservation and/or management schemes.

Beginning as a planning tool within Sweden’s national road administration some 50 years ago, benefit-cost analysis (BCA) has come to be a pillar of the national transport policy because of subsequent strategic choices made by the national parliament. These choices made it necessary to widen the analysis of costs to include also externalities and a foregone conclusion was that efficient investment priorities should be made based on BCA. But no one asked whether the political decision makers or the BCA models were up to that task. This paper reviews the institutional framework and practice of BCA in Sweden for transport infrastructure investment, and considers design issues that have been and still are debated, such as whether the discount rate should include a risk term and how to account for the marginal cost of public funds. A main concern with BCA results is the underestimation of construction costs, making transport sector projects look better than they are. Several ex post analyses have established that a higher NPV ratio increases the probability of being included in the investment program proposal prepared by the agency. The requirement to let projects undergo BCA seems to make planners “trim” project proposals by trying to reduce investment costs without significantly reducing benefits. This relationship is weaker among profitable projects. Moreover, there is no correlation between rate of return and the probability of being included in the final program, which is established on political grounds.
Tolerability and efficacy of the monoaminergic stabilizer (−)-OSU6162 (PNU-96391A) in Huntington’s disease: a double-blind cross-over study

Objective
To evaluate the safety (primary objective) and efficacy (secondary objective) of (−)-OSU6162 in Huntington’s disease (HD).
MethodsIn a double-blind, cross-over trial, patients with HD were randomly assigned to start treatment on either (−)-OSU6162 or placebo. After 4 weeks, those patients who initially received active drug were switched to placebo for another 4 weeks, and vice versa. During the first week the (−)-OSU6162 dose was 15 mg twice daily, during the second week 30 mg twice daily, and during the last 2 weeks 45 mg twice daily. Motor, cognitive, mental and social functions were rated by the clinical investigator or by self-assessment, using established rating scales.
ResultsFifteen patients fulfilling inclusion and exclusion criteria completed the study. (−)-OSU6162 was well tolerated by all patients and no adverse effects were observed. (−)-OSU6162 treatment significantly improved the Short Form 36 Vitality score, mainly due to an improvement of the individual item ‘worn-out’ (VT3). In addition, an improvement of depressive symptoms was found using Beck Depression Inventory. In contrast to a general trend of improvement in several non-motor variables only small and non-significant differences between (−)-OSU6162 and placebo were found regarding motor functions.
Conclusions(−)-OSU6162 offers promise for the treatment of HD, as a drug with good tolerability, capable of improving the patients’ experienced non-motor functions such as energy and mood and thus alleviating symptoms of great importance for their quality of life.

Objective
To examine the use of vitamin D supplements during infancy among the participants in an international infant feeding trial.
DesignLongitudinal study.
SettingInformation about vitamin D supplementation was collected through a validated FFQ at the age of 2 weeks and monthly between the ages of 1 month and 6 months.
SubjectsInfants (n 2159) with a biological family member affected by type 1 diabetes and with increased human leucocyte antigen-conferred susceptibility to type 1 diabetes from twelve European countries, the USA, Canada and Australia.
ResultsDaily use of vitamin D supplements was common during the first 6 months of life in Northern and Central Europe (>80 % of the infants), with somewhat lower rates observed in Southern Europe (>60 %). In Canada, vitamin D supplementation was more common among exclusively breast-fed than other infants (e.g. 71 % v. 44 % at 6 months of age). Less than 2 % of infants in the USA and Australia received any vitamin D supplementation. Higher gestational age, older maternal age and longer maternal education were study-wide associated with greater use of vitamin D supplements.
ConclusionsMost of the infants received vitamin D supplements during the first 6 months of life in the European countries, whereas in Canada only half and in the USA and Australia very few were given supplementation.
Potential Influence of Diet on Bomb-Pulse Dating of Human Plaque Samples

The radiocarbon concentration of different atherosclerotic plaque fragments obtained from 20 patients in Portugal, operated in 2000–2001, has been measured in order to define the year of plaque formation. A difference of 1.8–15 yr was observed, with the mean and median both 9 yr, between the bomb-pulse date estimated with the CALIBomb software and the operation date. Stable isotope (δ13C and δ15N) analysis was also performed and provides insight to the diet of the subjects. The wide range of measured stable isotope values could indicate that the subjects' diet varied, including an abundance of marine foodstuffs. It could also mean a different isotope fractionation process for the different plaque fragments (cap, core, interface to media) and a possible difference in tissues in which the various fragments are formed. Analysis of δ13C and δ15N values of each patient separately revealed subjects considered more influenced by marine foodstuffs consumption.
Evaluation of self-feeders as a tool to study diet preferences in groups of Atlantic cod (Gadus morhua)

Among other applications, self-feeding has been used to study food preferences in fish allowing them to choose between feeders with different food content. Preference tests assume that (i) trigger actuations are motivated by appetite, (ii) fish can learn which feeder contains which food and discriminate between feeders solely on the basis of their content, and (iii) in groups of fish, the triggering preferences is representative for the individuals of the group. We studied individual triggering behaviour in four groups of 14 Atlantic cod (length of 34 ± 2 cm, weight of 424 ± 102 g, mean ± SE, water temperature comprised between 7–8 °C) that were first given the choice between two self-feeders with identical content (Period 1 of 14 days) and subsequently with one feeder full and the other empty (Period 2 of 14 days) . In all four groups, one or two individuals performed the majority of the actuations, and in three groups the high triggering fish was a female high-ranked for size and growth rate. Cod displayed a preference for one of the two feeders despite their identical content. When the preferred feeder was emptied, the preference switched after one to eight days but both feeders were still actuated throughout the experiment. In conclusion, the assumption that actuation frequency reflects food preference and is representative for the individuals of the group may not be true, at least for Atlantic cod. If aiming at determining preferences representative for the whole population multiple representative fish should be kept isolated in separate tanks, with self-feeders containing each food option, on each tank.
Scoring models of a diet quality index and the predictive capability of mortality in a population-based cohort of Swedish men and women

Objective
To examine how different scoring models for a diet quality index influence associations with mortality outcomes.
DesignA study within the Malmö Diet and Cancer cohort. Food and nutrient intakes were estimated using a diet history method. The index included six components: SFA, PUFA, fish and shellfish, fibre, fruit and vegetables, and sucrose. Component scores were assigned using predefined (based on dietary recommendations) and population-based cut-offs (based on median or quintile intakes). Multivariate Cox regression was used to model associations between index scores (low, medium, high) and all-cause and cause-specific mortality by sex.
SettingMalmö, the third largest city in Sweden.
SubjectsMen (n 6940) and women (n 10 186) aged 44–73 years. During a mean follow-up of 14·2 years, 2450 deaths occurred, 1221 from cancer and 709 from CVD.
ResultsThe predictive capability of the index for mortality outcomes varied with type of scoring model and by sex. Stronger associations were seen among men using predefined cut-offs. In contrast, the quintile-based scoring model showed greater predictability for mortality outcomes among women. The scoring model using median-based cut-offs showed low predictability for mortality among both men and women.
ConclusionsThe scoring model used for dietary indices may have a significant impact on observed associations with disease outcomes. The rationale for selection of scoring model should be included in studies investigating the association between dietary indices and disease. Adherence to the current dietary recommendations was in the present study associated with decreased risk of all-cause and cause-specific mortality, particularly among men.
Sea caged Atlantic salmon display size-dependent swimming depth

The present study investigates vertical distribution of fish size in three 12 m × 12 m wide and 14 m deep sea cages stocked with Atlantic salmon of average weight from 3.5 to 3.7 kg, at commercial densities between 15.6 to 16.2 kg m-3, in Norway. For each cage, individual fish weight were estimated by three 0.6 m × 0.6 m measuring frames as fish swam through. The frames were positioned at 3, 6 and 9 m depth. Recordings were carried out over five days per cage in succession, and during relatively stable environmental conditions in late autumn 2010. In all cages, measured fish were 15−25% smaller at 3 m compared to the average weights at 6 and 9 m depth. Largest average weight difference between depths within one cage was 0.995 kg. The average weight at 6 and 9 m was higher at night-time compared to daytime. Fish at the lower end of the weight spectrum were predominantly registered at 3 m, while fish at the higher end of the spectrum were mostly registered at the greater depths. Fish of average size were well represented at all three depths. The fact that smaller fish swam shallower may be ascribed to natural behavioural traits and introduce an important consideration in representative sampling within commercial sea cages. In biomass estimations and sea lice counts size-dependent vertical stratification needs to be accounted for.
High disaccharide intake associates with atherogenic lipoprotein profile

Increased plasma concentrations of small LDL particles denote an atherogenic lipoprotein phenotype (ALP) that is correlated with increased circulating TAG and reduced HDL-cholesterol. Principal component analyses of subfraction concentrations have previously been used in the Swedish population-based Malmö Diet and Cancer (MDC) cohort to identify three independent components, one pattern representing the ALP. The aim of the present study was to examine the associations between macronutrient intakes and the principal component representing the ALP. We examined 4301 healthy subjects (46–68 years old, 60 % women) at baseline in the MDC cohort. Dietary data were collected using a modified diet history method. Plasma lipoprotein subfractions were measured using a high-resolution ion mobility method. The principal component corresponding to the ALP was significantly associated with a higher intake of disaccharides, and inversely related to protein and alcohol consumption (P < 0·001 for all). The present findings indicate that the ALP may be improved by a low intake of disaccharides, and moderate intakes of protein and alcohol.
Ceylon cinnamon does not affect postprandial plasma glucose or insulin in subjects with impaired glucose tolerance

Previous studies on healthy subjects have shown that the intake of 6 g Cinnamomum cassia reduces postprandial glucose and that the intake of 3 g C. cassia reduces insulin response, without affecting postprandial glucose concentrations. Coumarin, which may damage the liver, is present in C. cassia, but not in Cinnamomum zeylanicum. The aim of the present study was to study the effect of C. zeylanicum on postprandial concentrations of plasma glucose, insulin, glycaemic index (GI) and insulinaemic index (GII) in subjects with impaired glucose tolerance (IGT). A total of ten subjects with IGT were assessed in a crossover trial. A standard 75 g oral glucose tolerance test (OGTT) was administered together with placebo or C. zeylanicum capsules. Finger-prick capillary blood samples were taken for glucose measurements and venous blood for insulin measurements, before and at 15, 30, 45, 60, 90, 120, 150 and 180 min after the start of the OGTT. The ingestion of 6 g C. zeylanicum had no significant effect on glucose level, insulin response, GI or GII. Ingestion of C. zeylanicum does not affect postprandial plasma glucose or insulin levels in human subjects. The Federal Institute for Risk Assessment in Europe has suggested the replacement of C. cassia by C. zeylanicum or the use of aqueous extracts of C. cassia to lower coumarin exposure. However, the positive effects seen with C. cassia in subjects with poor glycaemic control would then be lost.

Objective
To develop a diet quality index (DQI) that assesses adherence to the Swedish nutrition recommendations (SNR) and the Swedish dietary guidelines (SDG).
DesignA cross-sectional study within the Malmö Diet and Cancer (MDC) cohort. A diet history method collected dietary data, a structured questionnaire lifestyle and socio-economic information, and anthropometric data were collected by direct measurements. The index (DQI-SNR) included six components: SFA, PUFA, fish and shellfish, dietary fibre, fruit and vegetables, and sucrose.

SubjectsMen (n 4525) and women (n 8491) of the MDC cohort enrolled from September 1994 to October 1996.
ResultsFor participants with high DQI-SNR scores, nutrient and food intakes were close to recommendations. However, most of the study population exceeded the recommended intake for SFA (98 %) and few reached recommended intakes for dietary fibre (24 %), fruit and vegetables (32 %), vitamin D (18 %) and folate (2 %). A high DQI-SNR score was positively associated with age, physical activity, not smoking, past food habit change, education and socio-economic status. Individuals with high scores were more likely to have a diabetes diagnosis or experienced a cardiovascular event.
ConclusionsResults suggest that the DQI-SNR is a useful tool for assessing adherence to the SNR 2005 and the SDG in the MDC cohort. No index has previously been developed with the aim of evaluating adherence to the current dietary recommendations in Sweden. Further validation of the DQI-SNR, and evaluation of its utility, is needed.

Recent changes in the status of Steller's Eider Polysticta stelleri wintering in Europe: a decline or redistribution?

Steller's Eider Polysticta stelleri has a restricted arctic breeding range. The world population declined to c. 220,000 individuals in the late 1990s from an estimated 400,000–500,000 in the 1960s. The species has a limited global wintering distribution, occurring in marine habitats in north-east Europe, islands close to Kamchatka in Russia, and the eastern Aleutian Islands and south-west Alaska. European wintering numbers were estimated at 30,000–50,000 in the early 1990s, when the population was considered of favourable conservation status. Recent census data from the most important European wintering sites show annual declines of 8% in Norway since 1984, 9% in Estonia since 1994 and 22% in Lithuania since 1995, suggesting an overall 65% reduction in Europe. Counts in 1994 suggested that 30–50% of the European population wintered in Russia at that time. Current census data from Russia show similar declines along monitored sections of the Kola Peninsula wintering grounds since 1994. Accounting for trends in Russia, the current European wintering population could possibly stand at 10,000–15,000 individuals (a more than a 50% decline in 10 years), qualifying this population as Endangered under IUCN criteria. The changes in Baltic/Norwegian wintering numbers did not correlate with changes in the extent of ice-free marine waters in the Kola Peninsula/White Sea areas, but changes in annual numbers in Norway were correlated with winter North Atlantic Oscillation indices. Variation in annual numbers in the Baltic Sea correlated with projected number of juveniles among wintering birds. However, none of the possible causes discussed in this paper could fully explain the decline in Steller's Eider, confirming the need for comprehensive monitoring of the population throughout its winter range and for cohesive demographic monitoring to target effective conservation action.

Molecular dynamics simulations are widely used today to tackle problems in biochemistry and molecular biology. In the 25 years since the first simulation of a protein computers have become faster by many orders of magnitude, algorithms and force fields have been improved, and simulations can now be applied to very large systems, such as protein–nucleic acid complexes and multimeric proteins in aqueous solution. In this review we give a general background about molecular dynamics simulations, and then focus on some recent technical advances, with applications to biologically relevant problems.
Vertical migration and dispersion of sprat (Sprattus sprattus) and herring (Clupea harengus) schools at dusk in the Baltic Sea

In populations of herring (Clupea harengus) or sprat (Sprattus sprattus), one typically observes a pattern of schools forming at dawn and dispersing at dusk, usually combined with vertical migration. This behaviour influences interactions with other species; hence a better understanding of the processes could contribute to deeper insight into ecosystem dynamics. This paper reports field measurements of the dispersal at dusk and examines two hypotheses through statistical modelling: that the vertical migration and the dissolution of schools is determined by decrease in light intensity, and that the dissolution of schools can be modelled by diffusion, i.e. active repulsion is not required. The field measurements were obtained during 3 days in March at one location in the Baltic Sea and included continuous hydroacoustical monitoring, trawl samples, and hydrographical CTD data. Echogram patterns were analysed using the school detection module in Echoview® and local light intensities were calculated using a model for surface illuminance. The data and the analysis support that schools migrate upwards during dusk, possibly trying to remain aggregated by keeping the local light intensities above a critical threshold, that schools initiate their dissolution when ambient light intensity drops below this critical threshold, and that fish subsequently swim in an uncorrelated random walk pattern.
On the Prevalence, Diagnosis and Management of Lithium-Induced Hypothyroidism in Psychiatric Patients

Fifty-three psychiatric patients who had been receiving treatment with lithium continuously for more than two years were examined to estimate the prevalence of lithium-induced hypothyroidism. It was found to be 20 per cent among women. No men were affected among these patients. In order to study the characteristics of the disorder further cases were drawn from another population. One third of the patients developed hypothyroidism during their first year of treatment, others not until after 9 years. About two thirds of the female patients with hypothyroidism had thyroid antibodies. All cases with lithium-induced hypothyroidism showed elevated levels of serum thyrotropin, which in our experience is the laboratory examination of choice in these as well as other cases of ‘primary’ hypothyroidism. Since the probability of detecting these cases at a given control visit was found to be low, we feel that such visits need not include extensive laboratory investigations. Hypothyroid patients responding well to lithium treatment should continue their medication combined with appropriate thyroxine substitution.
